Telehealth & Chronic Care Management

There are many types of visits that are well-suited for virtual care. With the coronavirus pandemic forcing you to change the way you work, you may need to rethink how some of your visits could be done virtually. Telehealth
  • Triage and Treat Minor Illness and Injury - using Telehealth for answering questions is an ideal way to determine the severity of a patient’s condition.
  • Follow Up After an Office Visit - After your initial office visit, a virtual follow up is an excellent opportunity to check on your patient.
  • Discuss Lab and Imaging Results - Virtual visits are an ideal way to share test results from a recent office visit. You can interpret results for your patients, discuss your plan, and make any adjustments to medications with electronic prescribing.
We combine our other remote care services Chronic Care Management (CCM), Transitional Care Management (TCM) and Annual Wellness Visits (AWV) with our integrated Telehealth module.

Our Process

  • We always start with our initial discovery call and in this case a demo of the service. Once you are ready to sign up, the following info will need to be collected to make the onboarding process quick
  • and as smooth as possible.
  • Onboarding questions and registration info:
  • How do you plan on utilizing Telehealth?
  • Why are you choosing telemedicine now?
  • What does your patient demographic look like? (age, medicare/medicaid, etc.)
  • Do you plan to bill for telemedicine visits? If yes, will you be billing through us or your EMR?
  • What EMR do you currently use?
  • How comfortable are you with telemedicine technology?
  • What do you envision your workflow to look like?
  • Who is the best point of contact to receive welcome emails, phone calls, discuss account details?
  • Do you have a dedicated technical contact we can work with as well?
  • Once those questions are answered, the following info will need to be collected to finish registration.

  • Name, PH#, email and NPI of all providers planning to have a Telehealth license. Once that info is collected a docusign will be sent to the main contact of the practice for review and signature. The docusign states financial terms agreed upon as well as payment schedule. Once Docusign has been reviewed, signed and returned a dedicated customer success manager will be reaching out to schedule the onboarding and implementation process.
